You have the option to travel from major cities in the US, Canada, UK and Europe and catch a connecting flight in one of four "gateway" islands: San Juan, St. Kitts, St. Maarten or Antigua.
Once you arrive at a "gateway" island, there are a number of small airlines that fly the next short flight onward to Nevis.
